Drivers History Report

To receive the personal information section contained in the Motor Vehicle Record, a registered user must complete a Personal Information Release Agreement with the DMV in addition to the Tyler South Carolina Subscriber Agreement and documents required by DMV. You may obtain the DMV's Personal Information Release Agreement and document checklist from Tyler South Carolina.

To be added as a subscriber, you must:

1. Download, complete, and sign the following:
   • Tyler South Carolina Subscriber Agreement (PDF)
   • Data Transaction Request Form (PDF)
   • Personal Information Release Agreement (PDF)
   • PIRA Addendum (PDF)
2. Return the documents to Tyler South Carolina by email, fax, or mail.
